IV Dose:
Loading dose:
2.0 - 2.5 mg/kg IV
Maintenance dose:
(to administer 12 h after loading dose)
1.25 - 1.5 mg/kg IV q12h
Intrathecal (IT) dose for Meningitis:
5 mg IT q24h x 3-4 days, then
5 mg IT q48h

ALTERNATIVE DOSING (BLUE BOOK / HSgB DRUG LIST)
Infants <2 years: IV up to 40,000 units/kg/day divided by every 12 hours. Intrathecal: 20,000 units/dau for 3-4 days, then 25,000 units EOD for 2 weeks after CSF cultures and glucose are negative or within normal range. Child > 2 years and adult: IV 15,000 - 25,000 units/kg/day divided every 12 hours. Intrathecal: 50,000 units/day for 3-4 days then EOD for 2 weeks after CSF cultures and glucose are negative or within normal range. Max dose: 2,000,000 units/day
