1. Nosocomial pneumonia
2. Bacterial Meningitis
3. Empirical treatment for presume infections in patients (adult & children) with febrile neutropenia, used as monotherapy or in combination with anti-virals or antifungal agent.
4. Septicaemia
5. Urinary tract infection
6. Intra-abdominal infections
7. Gynaecological infections

Usual dose:
1,000 mg IV q8h
Severe infections, MDR Organisms or CNS infections:
2,000 mg IV q8h
* Consider extended infusion.

1. Original dose: 1,000 mg IV q8h
CrCl 30-50 ml/min:
1,000 mg IV q12h
CrCl 10-29 ml/min:
500 mg IV q12h
CrCl <10 ml/min or HD:

2. Original dose: 2,000 mg IV q8h
CrCl 30-50 ml/min:

or
