INDICATION (BLUE BOOK / UKK)

Treatment of: (1) Oral or oesophageal candidiasis. (2) Fluconazole resistant and/or oesophageal candidiasis.

DOSING FOR ADULTS GUIDE [DIAMS HSgB]

Usual dose:
100-200 mg PO q12h

Consolidation therapy for cryptococcal meningitis, histoplasmosis or penicilliosis:
200 mg PO q8h for 3 days,
followed by 200 mg PO q12h

Maintenance therapy / secondary prophylaxis for cryptococcal meningitis / histoplasmosis / penicilliosis:
200 mg PO q24h

Treatment of Oropharyngeal candidiasis (oral thrush) / Oesophageal candidiasis:
200 mg PO q24h

Pulse regimen for Onychomycoses:
200 mg PO q12h x 1 week per month

50% of normal dose PO q12h

HD:

ALTERNATIVE DOSING (BLUE BOOK / HSgB DRUG LIST)

i) 200 mg daily for 1 week. If no response after 1 week, continue treatment for another week ii) 100 - 200 mg twice daily for 2 weeks. If no response after 2 weeks, continue treatment for another 2 weeks. The 400 mg daily dose should not be used for more than 14 days if there are no signs of improvement
