INDICATION (BLUE BOOK / UKK)

Treatment of acute uncomplicated lower urinary tract infections (acute cystitis) in females of 18 years of age and older caused by multidrug resistant (MDR) Escherichia coli or Enterococcus faecalis who are candidates for carbapenems or colistin.

DOSING FOR ADULTS GUIDE [DIAMS HSgB]

Uncomplicated UTI:
3 gm sachet PO x 1 dose

Complicated UTI:
3 gm sachet PO every 2-3 days x 3 doses

RENAL DOSE ADJUSTMENT IN ADULTS GUIDE [DIAMS HSgB]

1. Uncomplicated UTI:
No dosage adjustment

2. Complicated UTI:
CrCl 10-50 ml/min:
Limited data.
Consider 3 gm sachet OI every 3 days.

CrCl <10 ml/min:
No data, given prolonged half-life consider dose adjustment or avoid due to decreased urinary excretion.

HD:
Redose after dialysis, due to removal during HD.
