Febrile neutropenia, septicaemia, lower respiratory tract infection, urinary tract infection, skin and skin structure infections, gynaecologic and intra-abdominal infections
DOSING FOR ADULTS GUIDE [DIAMS HSgB]
Mild to moderate infections, UTI:
1 gm IV q8-12h
Intra-abdominal infection, Diabetic foot infection, SSTI, Osteomyelitis, Prosthetic joint infection:
2 gm IV q8-12h
Bacteremia, Pneumonia, Sepsis, Severe infections, Suspected/ confirmed Pseudomonas infection, CNS infections, Febrile neutropenia, Cystic fibrosis or Obese patients:
2 gm IV q8h

1. Original dose 1 gm IV q8-12h:
CrCl 30-60 ml/min:
1 gm IV q12h
CrCl 10-29 ml/min:
1 gm IV q24h
CrCl < 10 ml/min:
500 mg IV q24h
HD
1gm once, then 500 mg IV q24h

2. Original Dose 2 gm IV q12h:
CrCl 30-60 ml/min:
1 gm IV q12h or 2 gm IV q24h
CrCl 10-29 ml/min:
1 gm IV q24h
CrCl < 10 ml/min:

HD
1 gm once, then 500 mg IV q24h

ADULT:
1 - 2 g twice daily for most infections.
For severe infections including febrile neutropenia: 2 g 3 times daily.
CHILD:
2 mth - 16 yr: ≤40 kg: 50 mg/kg every 8-12 hr for 7-10 days
