(1) Infections caused by susceptible organisms.
(2) Infective endocarditis.

Leptospirosis
1.5-2 million units IV q6h

SSTI:
2–4 million units IV q4-6h

Infective endocarditis (Penicillin susceptible Streptococci):
3 million units IV q4h

Infective endocarditis (Streptococci relatively resistant to Penicillin, NVS), Neurosyphilis, Ocular syphilis or CNS Infection:
4 million units IV q4h

RENAL DOSE ADJUSTMENT IN ADULTS GUIDE [DIAMS HSgB]

1. Mild to moderate infection

CrCl <10 ml/min:
Administer a full dose, followed by 50% of the normal dose q6h

HD:
1 million units IV q6h


2. Serious infection

CrCl 10–50 ml/min:
2–3 million units IV q4h

CrCl <10 ml/min or HD:
2 million units IV q4–6h

ALTERNATIVE DOSING (BLUE BOOK / HSgB DRUG LIST)

i) ADULT:
600 mg - 3600 mg (1 - 6 mega units) daily, divided into 4 to 6 doses. Higher doses (24 mega units) in divided doses may be given in serious infections such as meningitis.

CHILD:
1 month to 12 years old: 100 mg/kg/day in 4 divided doses, not exceeding 4 g/day;
Infants 1 -4 weeks: 75 mg/kg/day in 3 divided doses;
Newborn Infants: 50 mg/kg/day in 2 divided doses


ii) 7.2 to 12 g (12 - 20 mega units) may be given daily in divided doses
