Caution!
*There are 2 types of Ampho B in HSgB; Ampho B deoxycholate (conventional) & Ampho B lipid complex (ABLC). Please ensure that reference is made to the correct Ampho B.
*Liposomal Ampho B is not available in HSgB

Amphotericin B deoxycholate usual dose:
0.6–1.0 mg/kg IV q24h


Dosing for Opportunistic Infections:
Penicilliosis:
0.6-0.7 mg/kg IV q24h

Histoplasmosis
0.7-1.0 mg/kg IV q24h

Cryptococcal Meningitis:
0.7-1.0 mg/kg IV q24h

RENAL DOSE ADJUSTMENT IN ADULTS GUIDE [DIAMS HSgB]

No dosage adjustment necessary; consider alternative antifungals if renal insufficiency occurs during therapy despite adequate hydration.

ALTERNATIVE DOSING (BLUE BOOK / HSgB DRUG LIST)

ADULT:
0.25 mg/kg/day by IV infusion,gradually increase if tolerated to 1mg/kg/day.
Maximum in severe cases: 1.5 mg/kg daily or on alternate days.

For neonates, lower doses are recommended
